Your First Visit to Our Cary Practice

In order to serve you well, we ask that you take a moment, download the following forms and fill them out prior to your first visit: New Patient Form and Metabolic Assessment form. If you prefer, we can give them to you when you arrive and our staff will be more than happy to assist you. Please consider arriving 15 minutes prior to your scheduled appointment.

We strive to be very thorough and ask that you allow about an hour for your initial visit. Your visit will comprise of the following:

Chiropractic Consultation:

This is the time when we review your chief complaints and Metabolic Assessment form, conduct a full history interview, and discuss our Holistic Philosophy. Our primary goal in the consultation is to ask pertinent questions and then LISTEN! That’s the key.

Examination:

Based on the information learned in the consultation, a complete Chiropractic, Orthopedic, Neurological, Postural exam will be done, followed by an Acupuncture examination including a computer graphing analysis. Please note that X-rays are not standard protocol. They are taken only if indicated.

This will complete your first visit. Our Chiropractic Assistant (CA) will discus with you what services were done, schedule your second visit (Report of Findings) and begin to handle financial arrangements. As a courtesy to you, we will verify with your insurance any coverage’s you may have. Details of future charges, payments options and how coverage will apply will be discussed in detail at your next visit.
